Case Management

Case Management is "a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates the options and services to meet an individual's needs using communication and available resources to promote quality cost-effective outcomes" (National Case Management Task Force, 1992). 

Case Management works because it makes sure that the patient has the appropriate level of care at the appropriate time at the highest quality.  In conjunction with your physician, case managers are available in assisting and coordinating your hospital stay and your discharge.
